Bleeding gums could signal more than dental problems: Researchers identify possible heart health risk

New Delhi: For most people, bleeding gums are little more than an annoying sign that it’s time to brush more carefully or visit the dentist. But emerging research suggests the consequences of poor oral health may extend far beyond the mouth. Scientists have now uncovered evidence that a common bacterium responsible for severe gum disease may also contribute to a potentially life-threatening heart valve disorder, raising fresh questions about how closely oral hygiene and cardiovascular health are connected. While experts caution that the findings are still preliminary, they offer one of the strongest biological explanations yet for the long-observed relationship between gum disease and heart problems.

The new research, presented at the American Heart Association’s Basic Cardiovascular Sciences Scientific Sessions 2026, focused on Porphyromonas gingivalis (P. gingivalis) – one of the primary bacteria behind chronic periodontitis, an advanced form of gum disease.

Researchers from Fuwai Hospital in Beijing examined tissue samples from patients undergoing surgery for calcific aortic valve disease (CAVD), also known as aortic valve stenosis. They found that diseased heart valves contained approximately 30 times more P. gingivalis than healthy valves, suggesting the bacterium may play a direct role in the progression of the disease rather than merely being present by chance.

Why is aortic valve stenosis so dangerous

Calcific aortic valve disease develops when calcium gradually builds up on the heart’s aortic valve, causing it to stiffen and narrow. As the valve becomes less flexible, the heart must work significantly harder to pump blood throughout the body.

Initially, many people experience no symptoms. However, as the condition worsens, it can lead to:

  • Shortness of breath
  • Chest pain
  • Fatigue
  • Dizziness or fainting
  • Heart failure in advanced cases

Currently, there are no approved medications that can stop or reverse the progression of calcific aortic valve disease, and patients with severe disease often require valve replacement surgery.

How can bacteria in the mouth reach the heart

Scientists believe the process may begin when inflamed or bleeding gums allow bacteria to enter the bloodstream. Once inside the circulation, P. gingivalis appears capable of reaching the heart’s aortic valve, where it triggers inflammation. The new study found that the bacterium activates an immune signalling molecule called interleukin-1 beta (IL-1β).

This inflammatory signal causes heart valve cells to behave like bone-forming cells, encouraging calcium deposits to accumulate within the valve – a hallmark of calcific aortic valve disease.

Animal studies strengthened the findings

To investigate whether the bacteria were actually causing damage rather than simply being associated with it, researchers injected live P. gingivalis into healthy laboratory mice.

The results were striking.

The bacteria travelled to the animals’ aortic valves, triggering inflammation, calcium buildup and valve narrowing—even in mice that did not have high cholesterol, one of the traditional cardiovascular risk factors.

Meanwhile:

  • Mice treated with antibiotics before exposure developed significantly less valve damage.
  • Mice injected with dead bacteria showed virtually no valve calcification.
  • Blocking IL-1β greatly reduced inflammation and calcium deposition in both mice and laboratory-grown human heart valve cells.

Together, these findings point to a biologically plausible mechanism through which gum disease bacteria could contribute to heart valve disease.

Does this prove bleeding gums cause heart disease

Not yet.

Experts emphasise that the study does not establish a direct cause-and-effect relationship in humans. The research has been presented at a scientific conference but has not yet undergone peer review, meaning independent experts have not fully evaluated the findings.

Dr. Elena Aikawa of Harvard Medical School, who was not involved in the study, noted that the results fit with the current understanding that calcific aortic valve disease is driven by inflammation rather than simply ageing or “wear and tear.” However, she stressed that gum disease is likely one of several contributing factors, alongside age, genetics, high blood pressure, cholesterol and other cardiovascular risks.

Oral-heart connection isn’t entirely new

Scientists have suspected for years that poor oral health may influence cardiovascular disease.

Previous research has shown that bacteria and inflammatory molecules from infected gums can enter the bloodstream, contributing to chronic inflammation elsewhere in the body. Severe periodontal disease has already been associated with higher risks of coronary artery disease, stroke and other cardiovascular conditions, although researchers have long debated whether the relationship is direct or explained by shared risk factors such as smoking, diabetes and poor overall health. The new findings provide one of the clearest biological mechanisms yet linking gum bacteria specifically to heart valve calcification.

The findings should not cause panic, but they do reinforce an important message: oral health is an essential part of overall health.

Dentists recommend seeking evaluation if bleeding gums persist for more than a few days, especially when accompanied by swollen, tender or receding gums or persistent bad breath. Maintaining good oral hygiene through twice-daily brushing with fluoride toothpaste, regular flossing or interdental cleaning, routine dental check-ups and prompt treatment of gum disease may not only help preserve teeth but could also reduce inflammation that affects the rest of the body.
