Morning heel pain is an incredibly common and frustrating issue that affects millions of people worldwide. If your very first steps out of bed in the morning are met with a sharp, stabbing pain under your heel, you are not alone. Understanding what causes this discomfort is the first step towards finding lasting relief. Dr Arun Oommen, Kochi-based neurosurgeon, explains. 

What is plantar fasciitis?
The most common culprit behind morning heel pain is plantar fasciitis. This condition involves the inflammation of the plantar fascia—a thick band of tissue that runs across the bottom of each foot, connecting your heel bone to your toes. This tissue acts as a vital shock absorber, supporting the natural arch of your foot and aiding in movement. When subjected to excessive stress, this ligament can develop tiny tears, leading to inflammation, stiffness, and that signature stabbing pain when you stand up.

Recognising the primary symptoms
The defining symptom of plantar fasciitis is a sharp pain on the underside of the heel, though it can sometimes extend to the middle of the sole. While it usually affects just one foot, it is not uncommon for both feet to suffer. The pain is typically at its worst during your first few steps after waking up or after periods of prolonged rest. Interestingly, active movement often temporarily eases the pain, but it can return with a vengeance after long periods of standing or when you finally sit down. Climbing stairs can also become exceptionally difficult due to stiffness.

Who is most at risk?
Plantar fasciitis often stems from the repetitive stretching and straining of the foot tissues. While anyone can develop it, active individuals aged between 40 and 70 are most susceptible. Statistically, women are slightly more prone to this condition than men, with pregnant women—particularly in their third trimester—frequently experiencing heel discomfort due to the sudden increase in weight and hormonal changes that loosen ligaments.

Key risk factors to watch out for
Several daily habits and physical characteristics can significantly increase your chances of developing heel pain:
1. Excess weight or obesity, which places immense, constant pressure on the plantar fascia ligaments.
2. Long-distance running or engaging in high-impact activities.
3. Occupations that require standing for long hours, such as teachers, restaurant servers, bus conductors, and traffic police officers.
4. Structural foot issues, including flat feet or unusually high arches, which alter how weight is distributed.
5. Having a tight Achilles tendon, which restricts ankle movement.
6. Wearing poorly cushioned shoes or regularly walking in high heels.

How is it diagnosed?
Diagnosing plantar fasciitis is primarily a clinical process. A doctor or orthopaedic specialist will examine your foot, locating tender areas and reviewing your medical history. While soft tissues do not show up on basic X-rays, imaging tests like X-rays or MRI scans are incredibly useful to rule out other potential causes of heel pain, such as stress fractures or calcaneal spurs (bone spurs). If the pain persists despite basic interventions, an MRI is highly recommended for a detailed look.

Effective treatment options
Managing heel pain begins with addressing the root triggers. Primary conservative treatments include resting the foot, applying ice packs, wearing night splints, and taking over-the-counter anti-inflammatory medications. Wearing custom orthotic shoes designed to redistribute pressure away from the plantar fascia is also highly beneficial. If conservative methods fail to bring relief, local corticosteroid injections—often administered under ultrasound guidance to ensure precision—can help dramatically reduce severe inflammation and pain.

Non-surgical therapies for recovery
Physical therapy is a cornerstone of recovery. A physiotherapist can guide you through targeted stretching exercises designed to lengthen the plantar fascia and the Achilles tendon, alongside strengthening routines to stabilise your gait. If the pain persists past six months, Extracorporeal Shockwave Therapy (ESWT) might be suggested. This non-invasive procedure uses high-energy sound waves to stimulate blood flow and accelerate healing in the damaged tissue.

When is surgery required?
In rare cases where chronic heel pain fails to improve after 6 to 12 months of consistent conservative treatment, surgical intervention may be considered. Surgery generally involves releasing a portion of the plantar fascia ligament to relieve tension. While plantar fasciitis is not a dangerous condition, its impact on daily mobility and quality of life can be severe. Seeking early clinical advice and adopting supportive footwear can help prevent the condition from worsening and help you walk pain-free.
