Who is most at risk of severe H1N1 infection? Doctor explains vulnerable groups and warning factors
New Delhi: Swine flu, or H1N1 influenza, does not affect everyone in the same way. While many people recover within a few days with rest and supportive care, others are far more likely to develop serious complications that may require hospitalisation. Pregnant women, young children, older adults and people with chronic medical conditions are among those who need to be especially cautious during the flu season.

Understanding who falls into these high-risk categories can help people seek medical attention early and take preventive steps before the infection becomes severe. In a communication with News9 Digital, Dr Namrita Singh, Director and Unit Head, Internal Medicine, Max Smart Hospital, Saket, explains which groups are most vulnerable to H1N1, what increases the risk of infection and why annual influenza vaccination remains the most effective preventive measure.
Who faces the highest risk?
Although anyone can contract H1N1 influenza, Dr Singh says the chances of developing severe illness are significantly higher in certain groups because of changes in immunity, age or underlying health conditions.
Pregnant women
Pregnancy naturally changes the immune system while placing additional demands on the heart and lungs. According to Dr Singh, this makes influenza infections more severe, particularly during the second and third trimesters.
Young children
Children younger than five years are more vulnerable to complications, with those under two years facing the highest risk.
“Their immune systems are still developing, making them more susceptible to complications such as pneumonia,” Dr Singh explains.
Older adults
As people age, their immune response gradually becomes less effective at fighting infections.
Dr Singh says adults aged 65 years and above are therefore more likely to develop severe disease and experience complications following H1N1 infection.
People living with chronic illnesses
Several long-term medical conditions can make it harder for the body to cope with influenza.
These include:
- Asthma and chronic obstructive pulmonary disease (COPD)
- Heart disease
- Diabetes mellitus
- Chronic kidney disease
- Chronic liver disease
- Neurological and neuromuscular disorders
- Certain blood disorders
According to Dr Singh, these conditions increase the likelihood of complications if a person becomes infected.
People with weakened immunity
Individuals whose immune systems are suppressed are also at greater risk.
This includes people receiving chemotherapy, those on long-term immunosuppressive medicines, organ or stem-cell transplant recipients, and individuals with medical conditions that weaken immunity.
“When the body’s natural defences are compromised, influenza can become much more severe,” Dr Singh says.
People with severe obesity
Dr Singh notes that severe obesity is also associated with a greater likelihood of influenza-related complications, making prevention especially important.
Residents of long-term care facilities
People living in nursing homes or long-term care centres face a higher risk because many residents are older, have underlying illnesses and live in close proximity, allowing the virus to spread more easily.
Healthcare workers and caregivers
Doctors, nurses, caregivers and other healthcare workers are frequently exposed to infected patients.
While their risk of severe illness depends largely on their own health and vaccination status, regular exposure increases their chances of contracting the virus.
What increases the risk of infection?
Dr Singh explains that H1N1 spreads mainly through respiratory droplets and aerosols released when an infected person coughs, sneezes or even talks.
She says the likelihood of infection rises in crowded or poorly ventilated places, during prolonged close contact with an infected person and when proper hand hygiene is not maintained.
“It is important to understand that the risk of getting infected is different from the risk of developing severe disease,” she says. “A healthy young adult may contract H1N1, but pregnancy, chronic illnesses, advanced age and weakened immunity are the factors that mainly increase the chances of serious complications.”
Vaccination remains the best protection
While practising good hand hygiene, maintaining respiratory etiquette and avoiding close contact with infected individuals help reduce transmission, Dr Singh emphasises that annual influenza vaccination remains the most effective way to protect against severe H1N1 illness.
She recommends that people in high-risk groups prioritise yearly flu vaccination to lower their chances of hospitalisation and serious complications during the influenza season.
